Diablo IV has so many potential villains. It’s a story about flawed people doing extremely flawed things, and then wallowing in self-loathing for hours after. But who’s the real villain? I’ll tell you, but beware! There are some endgame spoilers below!
Throughout the game, you chase after the seductive and oddly convincing Daughter of Hatred, Lilith. But as she sweeps through Sanctuary, turning more and more humans to her cause, it’s only natural to wonder who the real villain is. And for campaign purposes, the answer to that is Lilith.
When you descend into hell, you cut off Lilith before she can consume her father, Mephisto, one of the Great Evils who rules the Burning Hells. To do that, we capture Mephisto while he’s still weak in a soul stone and then fight an embittered and enraged Lilith in a two-stage boss battle. The first fight is against Lilith’s more beautiful form. The second fight is against a skeletal and serpentine version of her.

But as the name of the game suggests, it’s likely that we’ll encounter Diablo at some point in the game. The epilogue leaves plenty of open-ended questions to suggest there will be more encounters in the future. And if you look closely at the map, you’ll notice there are a series of Basins and Calderas which currently have no function. These are volcanic formations, and perhaps they have something to do with hell (and future enemies). We’ll just have to see!
