How to Get Sacred Items in Diablo 4

Sacred items are upgraded versions of items that drop in World Tier 3 activities such as the overworld, dungeons, events, you name it. Essentially, there’s a chance for every drop to become Sacred, meaning it’ll have an elevated item level as well as affixes that are exclusive to Sacred items.

Sacred items can be anything from Common up to Legendary, so just think of “Sacred” as a stat-boosting prefix. You’ll want to replace all your gear with Sacred gear during your time in World Tier 3 so you’ll be prepped to take on the level 70 Capstone Dungeon.

Well, if Sacred items drop in World Tier 3, you can probably guess where to find these. That’s right – World Tier 4 will begin dropping the even more prestigious Ancestral items. Think of these prefixes like a ladder – you’ll want to phase out the tier below as you seek higher and higher item level upgrades.

These items can drop from their respective World Tiers just about anywhere, but I’d strongly recommend Helltide Events and Nightmare Dungeons, as well as the occasional Tree of Whispers Bounty to target-farm ones you might be missing.
