The Milady is located in Castle Ensis, which is northeast of the Gravesite Plain region. Players can immediately reach there on horseback from the start of the expansion.

Warp to the Three-Path Cross Site of Grace in Gravesite Plain, then travel northeast over the Ellac Greatbridge. Keep heading straight past the castle defenses and there will be the Castle Front Site of Grace near the steps to the castle. This can take a few tries depending on your ability to dodge.

Elden Ring players can get the Milady by opening the chest in the left tower building of Castle Ensis. However, there are a few obstacles in the way to make it a bit harder.

Players going up the stairs from the Castle Front Site of Grace will be immediately welcomed by a literal giant blocking the path. You can take your time to defeat it or run past it, dodging his clubbing blows.

Run alongside the left side of the giant towards more stairs, where the path will wrap around the building and the giant will stop following. There will be a small campsite and cave entrance to the right, but continue running around the building opposite it to find wooden stairs up to it.

Take out the enemies that will fight you on the narrow stairs, then climb the ladder of the tower on the second floor to get the Milady from the chest. The light greatsword contains the Impaling Thrust Skill, which is a more powerful forward thrust attack, and requires 17 Dexterity and 12 Strength to wield.
